Albany Family Practice Group (AFPG) of Community Care Physicians (CCP) provides medical/physician services to the Albany College of Pharmacy students as part of their Infirmary Fee. This fee ($80 per year) is mandatory for all ACP students in years one through five. It is optional for sixth year students.

Albany Family Practice Group shall provide physician services for the Pharmacy students at the 2 Clara Barton Dr. offices, in Albany . The medical group will also provide 24-hour, seven-days-per-week on-call phone consultation coverage for any ACP student needing to speak with a physician. These services will be provided free of charge for ACP students under this agreement.

The services provided by AFPG will be for routine medical office care. The services will not include medications, prescriptions, medical testing, hospital care, emergency room care or any form of therapy treatments. Should these services be needed, the student's health insurance will be required and used as a method of payment for required services.

AFPG also provides "Convenience Care." This means that they are open daily from 7:15 a.m. to 10:15 a.m. and on Tuesdays and Thursdays from 5:15 p.m. to 6:45 p.m. for "walk-in" urgent appointments. Patients will be seen on a first come, first serve basis during these urgent visits.

It is understood that any additional clinical services may be required, from time to time, for the diagnosis or treatment of a student's medical condition. In these circumstances, the students will be referred to an appropriate practitioner by an AFPG provider. It is understood that these practitioners may or may not be members of AFPG and that health insurance coverage may be required by these providers at time of treatment or consultation.

Students should bring their Student ID Card and identify themselves as an ACP student when arriving at AFPG.
